I bring my own pedal board and Roland JC40 stereo amp - this can by mic'd up to FOH

Depending on the nature of the event I can alternatively bring an acoustic guitar which will require a DI to FOH

I bring my own vocal mic which I run through my pedal board - XLR output to FOH

I bring my own stomp box with built in microphone - this requires 48v.

Channels on mixer

Guitar/Banjo/12 string - either stereo or mono mics

Vocal - some processing will be sent from pedal board

Stompbox - I usually prefer this with some top end rolled off, more thud than thwack!
